PATIENT INFO

INFORMATION FOR NEW AND CURRENT PATIENTS

We are always thrilled to meet new patients in our office. If you’re planning an appointment at Butterfoss Barton Orthodontics, our team will take you through a thorough examination and consultation regarding your goals and orthodontic needs. You never know when diagnostic records may be necessary, so your appointment will have time built in to allow for any X-rays, photos or impressions of your teeth that our team may need to develop the right orthodontic treatment for you.

Patient Forms

For your convenience, we have provided the new patient info and below are downloadable forms. Simply download, print and fill out the applicable form, then bring the completed form in with you for your first appointment.

What Can You Expect From Your First Visit?

When you arrive for your first visit and consultation, our team will have a series of questions for you regarding your oral health, treatment goals, and any problems you may be experiencing prior to your visit. We will also seek to address the following questions to cover the basics of your orthodontic treatment needs. The following questions will often be answered or at least discussed during your visit. Of course, don’t hesitate to ask any additional questions you may have for us as well!

  • Are you currently experiencing any condition that an orthodontist can address?
  • Is treatment necessary now, or should treatment be delayed until some later time depending on the nature and progression of the issue?
  • What treatments will be necessary to rectify this problem?
  • Are tooth extractions required to ensure proper orthodontic health?
  • How long will the treatment take, and how much will the treatment cost?
  • What payment option is best for you?

The answers to these questions are usually clear by the first visit, but we will always address specific areas for deeper analysis or treatment based on your concerns, records, and smile goals. After this initial visit, we will often schedule a follow-up consultation to discuss options, time frames and financial arrangements for your individual treatment plan. These follow-ups may be in person, but they can also be conducted via telephone. This allows you additional time to ask any more questions you may have about your treatment plan.

What To Bring/Prepare For Your First Visit:

Your first consultation and exam with us are intended to give us all the information we need to create a personalized treatment plan that meets your orthodontic needs and smile goals. The most important thing you’ll bring to your appointment is, of course, yourself! However, there are some additional things you can provide at the first visit that will help our team give you the best possible care.

  • Any panoramic X-ray taken within the past six months
  • Your orthodontic insurance card, if applicable
  • A completed new patient form (if you chose to download and fill it out before your appointment)
  • Your questions!
